Our clinic doesn't retain personal or communication information about you unless you have chosen to receive service at Spine & Sports Centre. All information is collected and maintained with great diligence, using secure servers, in compliance with all current State and Federal privacy legislation.
For care at our clinic, we will ask you to volunteer personal details so that we can identify your specific needs, allocate appropriate appointments and communicate effectively with you regarding your care. We may also correspond with you via telephone, email, SMS or mail.
A proportion of our patients come via referral from medical or health practitioners. As such, it is standard practice to correspond with your medical practitioner where appropriate, which is only done with your express permission, and only between accredited professionals. These professionals require a minimum set of information to provide accurate identification of individual patients; particularly: patient name, date of birth and address. Reports to a Third Party, such as an insurer or lawyer, is only provided on our receipt of a Release of Information Document, signed by you.
All members of our staff adhere to a strict confidentiality protocol which extends within, and beyond their time of employment with Spine & Sports Centre.
